To me, this is kind of a metaphor for death, the black coat would be the grim reaper's cloak and the red hand will be a bloody hand. It would be a representation of death in the desert of loneliness that is the state of death, and when it says that it whispering his name, it would be the "wind of death" which whistles and comes to carry you.
